We make it simple and easy for you. You can download the New Patient Form here, or we can mail them to you in the post. Either way, all you need to do is fill out the forms and return them via email or by mail (we’ll even give you a stamped self-addressed envelope to make it even easier and hassle-free).
We’ll send you an SMS 2 weeks before your appointment to remind you what day and time your upcoming appointment is. We’ll send you another SMS 2 days before your appointment for you to confirm your appointment. Easy!

Private Health Insurance
Patients with private dental insurance are able to claim their treatment only paying the ‘gap’ using the health industry claims and payments service (HICAPS). Simply bring your insurance card with you to each visit and we can do the rest.
You can check if your private health insurer is part of the HICAPS system at their website here.
Changing or Cancelling appointments
When making your appointment please be aware we require 24 to 48 hours notice to change or cancel your appointment. Short notice changes or cancellations could incur a fee.
